Case Manager - Family Resource Center

POSITION SUMMARY:

The Case Manager acts as an advocate for Maryville residents, supporting the residents in maintaining their sobriety and in exercising their rights, choices, and responsibilities. The Case Manager acts as a single point of contact for all parties involved in the client’s well-being and care, with the goal to facilitate care coordination. The Case Manager also assists in the monitoring of program development and recovery planning in order to ensure a safe and nurturing environment. This position requires advanced knowledge in the field of child welfare and consistent exercise of discretion and judgment.

ESSENTIAL FUNCTIONS:

  • Provides frequent face to face contact and social services, ranging from routine to complex, in accordance with the terms agreed upon in the Individual Recovery Plan; identifies reports and recommends changes in resident services;
  • Completes a comprehensive assessment or reassessment of the Individual Recovery Plan of the assigned resident within the time frame established by the designated agency and develops a written recovery plan for each assigned resident;.
  • Prepares court reports, attends court hearings (as needed) and carries out orders of the court as they pertain to resident services;
  • Completes the resident intake forms
  • Assist residents in the transfer of school records and employment history; schedules resident in conferences with other professionals for purposes of case planning;
  • Provides hands on information to community resources (schools, employers, police and community agencies). Assists the residents in the utilization of available resources by providing linkage to appropriate resources;
  • Obtains timely school and job reports regarding the residents and provides information to appropriate authorities;
  • Completes the initial assessment within 24 hours and Recovery Plan within fourteen working days of the resident admission. Reviews initial Recovery Plan with the DCFS caseworker.;
  • Monitors and organizes appointments and transportation for residents to attend required appointments;
  • Maintains and ensures that case files are organized and contain all necessary information;
  • Reports all unusual incidents immediately to the supervisor. Follows up with written reports to DCFS as required within forty-eight hours;
  • Helps the residents set, develop and attain realistic personal, educational, social and employment goals;
  • Organizes, schedules and facilitates regular and emergency clinical staffings, ensuring notification and invitation of all required stakeholders. Documents staffings in accordance with program protocols, and within required timeframes;
  • Participates in staffings, staff meetings, treatment team meetings and in-service training sessions;
  • Documents all case management activities on Program Service Document Records;
  • Completes compliant Program Service documentation for those case management services delivered to the resident;
  • Ensures the Quality Utilization Reviews are completed in a timely manner, and that recommendations from reviews are implemented in a timely fashion;
  • Ensures that CFS440-5, CFS440-6, CFS440-7 forms are completed and received from DCFS within required timelines when a resident is accepted into the program;
  • Approves passes and visitations for residents in accordance with their programming goals;
  • Coordinates discharge planning for the resident from the time of intake, including the formulation, modification, and upkeep of the Recovery Plan;
  • Ensures the coordination and completion of the Transition and Discharge Protocol, including documentation and scheduling of post-transitional services;
  • Performs home visits at the Resource Center;
  • Participates in continued performance quality improvement; and
  • Other duties as assigned.

OTHER FUNCTIONS:

  • Communicates regularly with clinicians, program management, and the Program Director to ensure continuity and effectiveness of the recovery plan.
  • Transports residents in a Maryville van, as needed.

MINIMUM REQUIREMENTS:

Education/Experience:

  • Bachelor’s degree in human service related field.
  • One year residential care experience.
  • Minimum of 25 years of age

Physical Demands:

  • Required to frequently sit, stand, bend, stoop and walk, sometimes for extended periods.
  • Required to lift/ move objects weighing in excess of 20 pounds.
  • Must have visual, hearing, and learning capabilities sufficient to perform the essential functions defined above.

Environment and Scheduling:

  • Work is performed in designated site
  • Duties may involve contact with residents who may be threatening, aggressive and/or under the influence of drugs or alcohol.
  • Must be flexible and available to work additional hours when necessary.
  • May be scheduled to work on weekends and/or holidays

Salary range: $61,000-63,000 per year
